IJN Health Systems, LLC is a  Skilled Home Health Agency, is looking for a Licensed Practical Nurse (LPN)  to join our team of strong and compassionate caregivers. Our ideal candidate is a self-starter, motivated, and engaged.


Licensed Practical Nurse (LPN/ LVN) –Home Health  Home Care


Responsibilities:

  • Assess, plan, implement, and evaluate patient care.
  • Administration of medications and documentation
  • Health promotion, teaching, and training of family members
  • Wound care 
  • Patient assessments, coordination of care
  • Monitor and report changes in patient symptoms or behavior
Qualifications:

  • L.P.N, LVN or state-specific licensed nurse credential
  • Must be at least 18 years of age
  • Previous Home Care experience preferred, and a desire to serve and care for seniors.
  • Must be licensed and in good standing (LPN) in the state of Delaware
  • Minimum of six (6) months of experience working with seniors in assisted living, home health, independent living, hospital or long-term care environments and a desire to serve and care for seniors
  • Ability to react and remain calm in difficult situations
  • Ability to demonstrate good judgment, decision-making skills, and problem-solving.
  • Possess written and verbal skills for effective communication and a level of understanding
  • A driver's License and good access to transportation
  • CPR
